• End User Certification Statement (PDF): "We certify that the items purchased will be used by ___. The material will not be sold to a third party, distributed or used for any other purpose."
• For Diazepam Orders: Copy of DEA Registration Certificate for schedule IV drug: diazepam C-IV
• For Morphine Orders: DEA 222 Form with Quota Statement for schedule I or II drug: morphine C-II

Orders from hospitals/ambulance services:

• Pharmacy License
