  • DEA Form 236 (Controlled Substances Import/Export Declaration) Schedule C-IV drugs
  • Import Permit, non-morphine (Import Permit.pdf)
  • Letter of Authorization for Permission to Import, non-morphine (Authorization Letter.pdf)

For Morphine Orders:

All Purchase Orders should include:

  • Shipping Address
  • Billing Address
  • Freight: Responsibility of the buyer
  • Terms: CIF or Ex Works, Net 30 for all products
  • Letters of Credit or Deposits may be required depending on transaction
  • Taxation status and rate
