	IMPORTANT EXPORT INFORMATION: Auto-Injector are subject to the U.S. Federal Export Control Regulations and may not be exported or otherwise removed from the U.S. without prior written authorization from the U.S. Department of State.

• Form, DSP-83, signed original (Form DSP-83.pdf)
• Consignee or Forwarding Agent
• Port of Exportation/Port of Entry
• Route of transportation/carriers

For Diazepam Orders:

• DEA Form 236 (Controlled Substances Import/Export Declaration) Schedule C-IV drugs
• Import Permit, non-morphine (Import Permit.pdf)
• Letter of Authorization for Permission to Import, non-morphine (Authorization Letter.pdf)

For Morphine Orders:

• DEA Form 161 (permit to Export Controlled Substances) Schedule C-II drugs
• Import Permit, morphine (Morphine Import Permit.pdf)
• Letter of Authorization for Permission to Import, morphine (Morphine Authorization Letter.pdf)
  
  

All Purchase Orders should include:

• Shipping Address
• Billing Address
• Freight: Responsibility of the buyer
• Terms: CIF or Ex Works, Net 30 for all products
• Letters of Credit or Deposits may be required depending on transaction
• Taxation status and rate
